Transcripts

The official transcript(s) you submit as part of your application for admission:

  • Must include documentation of all post-secondary course work completed for which college credit was awarded, including course work completed during high school (except for AP credit).
  • Must be sent directly to Ohio State from each institution where credit was received.

If you've completed course work at Ohio State:

Current and former Ohio State students are encouraged to upload a copy of their Ohio State advising report — or a transcript, if they have one — to their online application or via the Admissions Uploader.

  • Former students who do not have access to these documents do not need to submit transcripts for course work completed at Ohio State.
  • If you are applying to public health, physical therapy or another program that uses a Centralized Application Service (CAS), such as AADSAS, ATCAS, AMCASCSDCAS, OptomCAS, OTCAS, PharmCAS, SOPHAS or VMCAS, you may be required to have an official Ohio State transcript sent to the application service. Consult your program for more information.
